All players requesting a Permission to Skate Form must be registered with SDMHA prior to the form being processed. Please read the Registration Information Sheet on the home page of the website and use the link located in the ‘registration tab’ to access our on-line registration system and manual. Cheques for registration will be held as a courtesy until notified of the status of the player at which point the cheques will be returned or cashed.
